Terms and Conditions

  • Each eGift has a unique code that corresponds to a service, item, or package. To use the eGift at the clinic, the code must be given by the eGift holder at the reception for validation. A person must be at least 18 years of age.
  • To redeem eGifts for Complete Eye Check-up for Senior Citizens and Cataract Screening for Senior Citizens, patients should be at least 60 years of age, and should hold Filipino citizenship and at least 6-month residency in the Philippines.
  • Any test outside of the service package that is conducted on the patient shall be charged to the account of the patient at the time of visit at the clinic.
  • Additional tests for Cataract Screening and Cataract Screening for Senior Citizens may be required, depending on the eye condition.
  • Eye doctors are pre-assigned to holders of the EyeSite eGifts.
  • The eGifts for Cataract Screening and Cataract Screening for Senior Citizens do not cover the consultation fees of the doctor and sanitation fees. eGift holders are advised to prepare for the cost of consultation and sanitation fees.
  • The eGift holder must wait for the attendant to finish the validation process. The attendant will receive a validation response indicating the eGift amount and validity. The attendant will follow only what is indicated in the validation response for communication to the eGift holder.
  • The eGift holder is responsible for the safekeeping of the eGift code. The eGift holder must only give this to the attendant once he/she commits to use it.
  • In the event that the eGift validation system is offline or unavailable, the attendant shall politely decline to accept the eGift. The eGift holder may still use the eGift when the system is back online.
  • The Peso denomination eGift may not be exchanged for cash. The service and product package eGifts may not be used to pay for other services than the one listed on the eGift face and may not be used in conjunction with other discounts offered by EyeSite Vision Studio.
  • The eGift is for one-time use only. If the total value of a purchase or bill is higher than the eGift value, the eGift holder is to pay for the difference. If the total value of an eGift is not used up, there will be no cash change given.
  • In the event that the price of a product or service package changes, the eGift corresponding to that product or service package may still be used.
  • The prices of service and product packages are subject to change without prior notice.
